Over 25 years ago, we began our practice in Middletown, NY --- the first in the region to offer Urgent Care services. Since 1984, we have grown rapidly into a multi-specialty physicians' group, while still offering Urgent Care on a walk-in basis.

In order to expand our Urgent Care services beyond Middletown into the Ulster and Sullivan county regions, we opened our Ellenville location. We now have Urgent Care services available to you and your family with no appointment necessary. Also, we have primary care and specialty services available on a rotating basis to continue to bring high quality, compassionate care to this area.

From NYS Route 17 West, take Exit 113 Wurtsboro at end of ramp make a right onto US 209. Proceed about 14 miles to Ellenville, turn right onto Shoprite Blvd. We are located at the bottom of the hill.

From NYS Route 17 East, take Exit 113 Wurtsboro at end of ramp make a left onto US 209. Proceed about 14 miles to Ellenville, turn right onto Shoprite Blvd. We are located at the bottom of the hill.

From Interstate 84, take Exit 4E to Route 17 East and follow directions above.
